Survodutide (10 mg Vial) Dosage Protocol
Reconstitution and weekly-titration guide for the 10 mg Survodutide vial — a GLP-1 / glucagon dual receptor agonist studied for weight management and metabolic (liver) health.
Quickstart Highlights
Survodutide (BI 456906) is a dual agonist of the GLP-1 and glucagon receptors. GLP-1 activity supports appetite control and insulin response, while glucagon-receptor activity is associated with increased energy expenditure and hepatic fat handling. It is studied for obesity and for metabolic dysfunction-associated steatohepatitis (MASH), and is dosed once weekly by subcutaneous injection with gradual titration.
- Composition: 10 mg Survodutide per vial
- Reconstitute: Add 2.0 mL bacteriostatic water → 5 mg/mL concentration
- Easy measuring: At 5 mg/mL, 1 unit (0.01 mL) on a U-100 syringe = 50 mcg (so 1 mg = 20 units)
- Storage: Lyophilized: −20 °C (−4 °F); reconstituted: 2–8 °C (35.6–46.4 °F), protected from light, use within 2–4 weeks
Dosing & Reconstitution Guide
Once-weekly subcutaneous titration to improve tolerability
Weekly Subcutaneous Protocol (10 mg vial, 2 mL = 5 mg/mL)
| Weeks | Weekly Dose | Units (U-100) | mL |
|---|---|---|---|
| 1–4 | 1 mg (1,000 mcg) | 20 units | 0.20 mL |
| 5–8 | 2 mg (2,000 mcg) | 40 units | 0.40 mL |
| 9+ | 4 mg (4,000 mcg) | 80 units | 0.80 mL |
Administration: Once weekly via subcutaneous injection, titrating upward slowly to limit gastrointestinal effects. Clinical research has explored maintenance doses in the low-single-digit milligram range. A 10 mg vial covers several weeks of the lower titration steps.
Reconstitution Steps
- Allow the vial to reach room temperature (15–20 minutes out of cold storage).
- Draw 2.0 mL bacteriostatic water with a sterile syringe.
- Inject slowly down the vial wall to avoid foaming.
- Gently swirl or roll until fully dissolved (do not shake).
- Label with date and concentration; refrigerate at 2–8 °C, protected from light.
Result: 5 mg/mL concentration
How This Works
Survodutide engages two receptors. GLP-1 receptor activity reduces appetite and improves glucose-dependent insulin secretion, while glucagon-receptor activity is linked to greater energy expenditure and mobilization of liver fat.
This dual action is why it is studied both for body-weight reduction and specifically for fatty-liver disease (MASH). Slow titration improves gastrointestinal tolerability.
Key Administration Notes
- Titrate gradually; gastrointestinal effects are most common early.
- Rotate injection sites; use new sterile insulin syringes and a sharps container.
- Once-weekly dosing — track the day to keep a consistent schedule.
- Document weekly dose, date, and site for consistency.
